| Fields marked with an asterisks * and in bold are required, all others are optional. | |
| FIELD | INFORMATION |
| *Last name | Must be the full last name of the applicant. |
| *First name | We prefer the full first name of the applicant, but
initials are acceptable as long as you provide a full middle name.
Examples: |
| Middle name | We prefer the full middle name of the applicant, but
initials are acceptable as long as you provide a full first name.
Examples: NOTE: Middle names are very useful for determining the correct person, and preventing mistakes. |
| Suffix | Jr., Sr., III, etc. Do NOT include professional suffixes such as "Esq." or "M.D." |
| *Bar Number | ATTORNEYS: Enter your Bar number. If your
state does not issue Bar numbers: enter N/A (for "not
applicable"). If you are an attorney who is NOT admitted to any state bar, but are employed or retained by the United States government or any of its agencies, enter N/A. Probation officers: enter "Probation" SMG Applicants (Journalists and interested members of the public): Enter SMG for your bar number |
| *State where admitted to the BAR | ATTORNEYS: If you are admitted to a state bar,
enter the 2-letter code for that state or territory (such as CA for
California). If you are NOT admitted to any state bar, but are an attorney employed or retained by the United States government or any of its agencies, or are Probation Officer, enter US. If you are a foreign attorney, or neither of the options above apply: enter NA (for "not applicable"). SMG Applicants (Journalists and interested members of the public): Enter the 2-letter code for your state or territory (such as CA for California). |
| *Date of admission to the bar of this Court | ATTORNEYS: Date (mm/yy) of when the
attorney was admitted to the bar (or granted pro hac vice) in the
Northern District of California, Federal Court. If you do not have the
exact date, your best guess is acceptable. If you have a pro hac vice application pending, or if you're registering in order to submit a pro hac vice application or apply for admission electronically, put the date you expect to be granted admission (generally, within a week of e-filing your application). | *Applicant's e-mail | The applicant's primary email address. Each applicant should have a unique email address. Do not use the same email address for multiple applicants. Secondary email addresses can only be added after the applicant receives the login and password. Instructions are available on the Signing Up for Case-specific Emails/NEFs page. |
| *Login | The login name you will use to access the ECF system.
Your login must be the LAST NAME of the applicant, Please use all lower case! Example for "Elmer Fudd": fuddzz99 Note: Your password will be generated by the system and emailed to you when your registration is complete. |
| *Key Question | The "key question" you provide here will be asked of you if you ever need to recover and/or reset your password. Choose something only you will know, but do not use sensitive information such as your mother's maiden name or social security number. |
